Transition Services
The Individuals with Disabilities Education Act (IDEA) requires that transition services be included in every student's IEP beginning at age 16 or earlier, when appropriate.
"Transition services" refers to a coordinated set of activities for a child with a disability. These activities are designed to improve academic and functional achievement and to facilitate the child's movement from school to post-school activities .
The foundation of the transition services plan is a coordinated effort among the student, parents, schools, and adult service providers to achieve a desirable future for the student.
The Louisiana Department of Education strives to help youth with disabilities and their families achieve desired post-school outcomes through student driven transition planning, interagency collaboration, technical assistance and training.
