The Early Childhood Transition Process

Early Childhood Transition for children with disabilities, ages 3-5, and their families is a carefully planned process of passage of a child and family from one set of services and programs into other services and programs, or in some cases, a transition out of service completely.

Transition Task Force – addresses the concerns regarding the transitioning of children with disabilities from early intervention programs, (ages 0 to 2), to early childhood school-based programs at age 3, and the transitions that take place during the early childhood years (ages 3-5).
